When implementing a population health model many organizations take an incremental approach rather than a complete transformation of the organization. This method often leads to limited success in achieving risk-based performance results. Producing results at scale and spreading across all risk populations requires a different way of thinking.
This session will describe a comprehensive framework used by multiple healthcare organizations as they implemented population health models. The five building blocks of the framework include designing a new model, building the new analytics engine, utilizing a nine-step approach roadmap for creating the new production system, building the learning organization and moving from prototype to full scale production.
At the end of this session, participants will be able to:
- Outline a comprehensive framework for transforming the organization and building the capacity and capability to succeed in a risk-based environment.
- Assess their own organization’s current state of capacity and capability to take on large scale risk across all payer populations.
- Develop an overall plan to implement the framework across five domains in their organization.
